Secondary providers collaborate with the Primary Service Provider (PSP) and the family, as members of the IFSP team, when discontinuation of the secondary treatment service is being considered. Services may be discontinued when the child transitions out of Part C at age three, when the family declines services, or when the IFSP team determines that a service is no longer needed. Decisions to discontinue services are made by the IFSP team and may occur at any time during the child’s enrollment in early intervention.
Secondary providers contribute to IFSP team discussions by providing information regarding the child’s progress, ongoing needs, and the medical or developmental necessity of continued intervention. When a secondary service is determined to be no longer necessary, the PSP will revise the IFSP or, at the annual IFSP review, update the services summary to reflect removal of the service.
Secondary providers are not required to complete any exit documentation but should assure all daily notes are completed. Child exit from early intervention is completed by the PSP and administrative staff. The PSP is responsible for notifying secondary providers when a child exits Part C services.
