Exited as “Attempts to contact unsuccessful”

Procedure
Never Seen, lost, and unavailable children enrolled more than 6 months:

1. Re-Evaluation within the last 3 months 
If the child was evaluated within 3 months of discharge, the objective findings tab will auto-populate the most recent IDA scores. The provider must complete:

  • Progress Note tab
  • Treatment Plan
  • Discharge
  • Charges tab: the discharge note was completed through file review, so add charges to FSC for time spent reviewing and completing the discharge note.
  • Child Outcome SummaryThese scores can be rolled from the re-evaluation, noting progress, if the Primary Evaluator completed the "Re-Evaluation" note type and completed the "Progress" tab.

2. Last evaluation more than 3 months before discharge 
If no evaluation has been completed in the last 3 months, the provider must supply IDA- 2 scores by:

  • Completing an IDA-2 review without the family present and recording the scores in the Objective Findings tab.
  • If the child was never seen, or only seen once or very briefly, enter “ntseen (+ space bar)” in the “Describe progress since last rating” box. This will auto-populate the note: 
    “This child was exited due to ‘attempts to contact unsuccessful’ and no therapeutic interventions were provided for the child, therefore developmental progress is without current observations.”
  • Charges tab: this discharge note was completed through file review, so add charges to FSC for time spent reviewing and completing the discharge note.
