It is PIC's practice to complete a discharge note for outgoing transfer children due to the fact that PIC is longer engaging the family in services and we could be held responsible for exit requirements with the state if they don't connect with the new ILP. It also acts as a final file review of the chart, by both direct service providers and admin, to ensure compliance and best practices are being followed.
Incoming Transfer: if the child was transferred to PIC from another ILP within the state, they are considered enrolled from the time they enrolled with the previous ILP. The child’s enrollment status date will be their ILP enrollment date and so even if PIC did not have the child enrolled for more than 6 months, the requirements are based off state ILP enrollment time.
Outgoing Transfer- if child is moving within the state of Alaska, complete the discharge note as directed, but choose the option “Transfer to ILP within AK”. Data entry will NOT add exit IDAs unless there is an issue with the family connecting to the new ILP.
